This population health quality improvement (QI) project by Dr.LeVance underscores the role of targeted prevention through effective implementation science with an improved discharge teaching strategy to reduce cerebral vascular accident (CVA) recurrence . The sub-population of CVA patients were isolated and interventions were applied to those identified as the highest-risk population. Through the lens of the Risk Population-specific Approach (RPsA), cognitive ergonomics were engaged to influence care outcomes via established discharge teaching bundle (DTB) juxtaposed to previous discharge practices. Respondents were provided a presurvey after previous discharge practices to measure effectiveness before intervention. By engaging the established DTB, results indicate 58% overall improvement with all respondents and a 300% improved CVA risk behavior change readiness with those of highest risk of CVA. Therefore, this project demonstrates that with Highest-risk Population Considerations (HrPC) there is strong potential to advance health equity through targeted prevention for the greater good of the general population by tailoring interventions to those who need it most.
